1. Appointment Calls

Appointment Confirmation: As a courtesy to our patients we use an award-winning automated communication system,Lighthouse 360. Lighthouse 360 will send emails, texts, postcards, or will call to ensure our patients are aware of their appointment’s time and date. Please confirm your appointment by texting “C” in response to a text or click the appropriate option in your email. Also you may call the office 814.336.4304 and our diligent front office staff will confirm your appointment.

2. Cancellations The office does not accept cancellations by voice mail during normal business hours. The office requires 24 hour notice for cancellations to avoid a missed appointment fee of $50.00.

3. Financial Agreements The office provides a financial agreement for any procedure over $200.00. Our financial experts will review the estimated insurance benefit and estimated copay. Patients are responsible for any balance not covered by insurance. Insurance copayments and self- pay payments are due day of service.

4. Changes Please notify the office of any of the following changes: phone, address, or insurance. Have your insurance card available at the front desk upon arrival and check- in. Also please notify the office if you have experienced medical changes or if pre-medication is required prior to dental appointments..
